Born in Ballarat, Victoria and a builder's labourer by trade, Samuel Richard Adams was 33 years, married and had three children. Was enlisted on 7 August 1915 in the 6th Battalion, 11th Reinforcement, Australian Imperial Force, with the rank of Private, and listed as Returned to Australia on 24th June 1917 for 6 months change. On 16th of May 2016 he received gunshot wound (G.S.W.) and on 22nd of May 2016 was admitted to hospital. Later on 26th of July 1916 was advised returning to Australia but on 8th August 1916 received a bomb wound to the head and discharged on 26th of January 2017. On 26th of February 2016 Samuel Richard Adams was transferred from 6th to 8th Battalion at Serapeum.
